1st Communion Sponsor Form

In order to be sponsor a person must meet the following requirements: Must be at least 16 years of age Must be baptized, confirmed and received First Communion If married, they must have been married by thte Catholic Church (Proof is required) In accordance to the Cannon Law, if the sponsor is single, they should not be living with anyone from the opposite or same sex The sponsor cannot be a parent or spouse of the candidate The person should be a strong role model and support the candidate by attending Mass regularly and by being active in some way in the Church
